Description
The form control is not correctly associated with its visible label either explicitly or implicitly.

Element name:

• User name edit field
• From date edit field
• To date edit field
• Performed Actions Combobox
• Event Category Combobox

Location of the elements: Form fields, combo boxes, under the "History" tab, in the main content.

The problem that it causes: Screen reader users are not able to identify the purpose of the form fields.
