Fix designer actions editor

In Designer

• Create an operation

=> Input field is shown twice with the same binding value
=> Label is not shown as mandatory and saving without a label works
=> When editing an existing entry for an operation the element field can be changed

Expected:

• input should only be shown once
• label should be mandatory and you should not be able to save without providing a value for it
• can't change the element when editing an operation
