Make CSV import for vocabulary creation user-friendly

The dialog pictured in the screenshot is verbose and confusing.

• The instructions should be shorter and clearer, maybe even hidden behind a because once you know how to use the feature it won't be necessary to read the instructions each time.
• There should be only one field in which to select the CSV file (currently there is the first Browse button for import and the second is the Select CSV button which is inconsistent).
• Once the CSV is selected, the user should be presented with the option to IMPORT or REFERENCE (the brief explanations for each could appear here or upon clicking a or "what's this?" link.
