Create comments on a placeless document must be effective

Since the new version of the comment manager implementation (TreeCommentManager),
the creation of a comments on a placeless document (a File for example) fails. This problem is due:

• To the fact that each document created under a placeless doc is a placeless too.
• We have a NPE exception because the parent of the commented document is null.
