Add ellipses and tooltip to document type on the initial create form

On the creation wizard, in the create tab, if a document type is too long it will degrade non-graciously as the full name will appear in a small space.

Solution
Use elipsed on the name and add a title to the whole item with the full name.

Also, expose item height and width (for change on theme).

Nice to have: if the name can still use two lines.
