Stream records need to be based on blobs not docs

Currently the DocStream class represents a document in its streamed format with blob attachments.

To process individual blobs in a stream we need to have each blob as a stream Record, the document metadata provides context but the blob is the "subject" of the data.

The same applies for binary text.
