Fix note format not displayed

When I create a note, no format is selected by default. If user doesn't pay attention, they can create the note without any format and get an error after clicking on Create:

null is not an object (evaluating 'this._blob['mime-type'].match')

You may have to create 2 or 3 notes for the Format field on the Note Creation form to come up blank.
