Clicking create multiple times on the same form creates duplicate documents

If the 'create' button is clicked multiple times before the form closes, multiple duplicate documents are created from the same form.

Clicking 'create' button should disable the button until the form is submitted so the user can't mistakenly create multiple duplicate documents.

In the video, you can see the arrow move over the 'create' button. I clicked the button 3x in the video before the form closed. This created 3 duplicate folders.
