Reuse common login page in DuoFactorsAuthenticator

      Description

      Currently this authentication plugin uses a custom login page duofactors.jsp that is a fork of login.jsp.

      A way to avoid this would be to use a generic page with JSP includes.

      Note that:

      • This page was added by NXP-14622.
      • Since NXP-20225 login.jsp has been refactored, so now there is a bigger gap between login.jsp and duofactors.jsp.
      • With NXP-19992 login.jsp will be moved to nuxeo-platform-web-common thus included in the nuxeo-server distribution.
