Add upgrade instructions for blob filename

When the blob was added a filename, it was not filled for existing documents.
It was was previously kept in a distinct field in case of file and files schemas, an upgrade instruction can be useful to add it back on the blob, see http://answers.nuxeo.com/questions/6923/problem-with-file-schemas-during-export-from-542-to-57
