Notifications settings action

I need a page to be able to select what notification channel I want for every kind of notification.

Notes from Lisa:

• I like the idea of getting to the settings page via a "notif" icon like in slack (see attachment). Ideally, you could get to your settings page OR manage the settings for a given doc from the same place (notice how you can adjust the channel settings from this modal, too?)
