Append suggestions instead of set on multivalue fields

On ML custom model suggestions.
When a document field is multi-value (ie. subjects), if we have several suggestions for that field:

First case
when the user clicks on a suggestion it should append to the field value (even if the field already has values).

Second case
If a user clicks again on the already added suggestion, it will remove only that suggestion
