Appearance of title field placeholder should be consistent when creating a new document

Steps to reproduce:

• Navigate to a folder
• Click on the Add Content button to create a new document
• Select any document type
  => Title field sometimes contains the placeholder Default title, sometimes nothing.

I have reproduced this with Audio, Video, File, and Note documents in Web UI, but not JSF UI.
