make LiveEdit bootstrap modules always generate and use nxedit:http:// or nxedit:https:// URIs

Update all components to get rid of the nxedit:// scheme and always explicitly specificy the protocol (nxedit:http:// or nxedit:https//). Also not that the same protocol handler registered for nxedit: will work for both URI style (no need to register two protocol handlers).

• DocumentModelFunctions (JSF functions to generate the URIs)
• FF protocol handler
• IE protocol handler (yet to implement)
